If your pet has been trapped or you know of any trapping incidents, please tell us. We would like to know:

• Your name & address
• Where & when
• The species trapped and the animal's name if available
• The kind of trap
• Any injuries
• Time spent in the trap
• Any medical treatment the animal received
• Was the incident reported to any agency?
• Was the trapper identified and did he receive a violation?
• Your comments
